These Terms of Service (Terms) set out the conditions that apply to the provision of services by Feel Good Physio Co (Physioformance Pty Ltd ABN 14 662 959 837) (FGPC, We, Us, Our) to you.
We provide physiotherapy services performed by qualified and registered physiotherapists (Physiotherapy Services).
This clause only applies to the provision of Physiotherapy Services.
By engaging Us to provide our Physiotherapy Services to you, you consent to Our physiotherapists undertaking assessments/examinations, treatments, manipulations, therapies, rehabilitation, and medical diagnostic procedures in compliance with their professional training and their understanding of your injury/condition.

By seeking Our services, you consent to FGPC collecting personal information about you and holding, using, disclosing or otherwise handling that personal information in accordance with FGPC's Privacy Policy.
All fees are due and payable when We provide Our Physiotherapy Services to you.
Our fees may change from time to time, but will be displayed at www.feelgoodphysioco.com.au.
Where you have booked an appointment with FGPC and wish to cancel that appointment with less than 12 hours' notice, you are required to pay the full appointment fee (Cancellation Fee).
Private health insurance, or government health insurance scheme or program benefits are not claimable on Cancellation Fees.
FGPC may, at its sole discretion, waive a Cancellation Fee.
Insofar as permitted by law, FGPC's liability for any injury, damage or loss (including consequential losses) arising out of any treatments or procedures provided by its employees, contractors or agents is capped at the total aggregate fees paid by the person suffering any injury, damage or loss arising out of any treatment or procedure rendered by FGPC's employees, contractors or agents.
Where you elect to cease treatment contrary to the advice of FGPC and/or its employees, contractors or agents, FGPC will not be liable for any injury, damage or loss arising out of your decision to cease treatment, including any consequential losses.
This limitation of liability does not impact any rights you may have under other laws such as the Australian Consumer Law.
These Terms are subject to the laws of South Australia. The parties submit themselves to the exclusive jurisdiction of the courts of South Australia and any competent appellate courts.
These Terms comprise the entire agreement between the parties in respect of the subject matter of these Terms.
